Standard Reptile Observation Form?

My daughter loves going out to find bugs and herps. I’d like to make the process more educational, and valuable for others, by bringing along some sort of standard observation form on which to collect basic data about our sightings; weather, measurements, habitat, behaviour, that sort of thing. Does anyone have a standard form they could send me, so I don’t have to totally reinvent the wheel? This will primarily be used with reptiles and amphibians, also insects and arachnids (maybe I need a couple of forms…).

Moon phase at night, cloud cover, and temperature of objects used for cover are some useful things to note
