Description of problem: Same user being able to vote twice on DQA
Step 1: Clicked on observation notification
Step 2: Scrolled down to update my DQA vote, noticed same user voted twice somehow
Step 3: Refreshed my page, updated my vote again and their votes remained.

Engineer took a look and deleted the extra vote. We still need to add some code that prevents this from happening, it’s likely a race condition.

Should be prevented from happening in the future.
