Quick little feature request for an addition to the Moderation History pages: Please make it so that when someone is suspended, whoever suspended them shows up in the moderation notes, and/or they are forced to add a reason for suspension.

Maybe this should also count for unsuspensions? I think the same issues of miscommunication happen in those cases as well.
