The page for adding observation seems to be missing elements. It seems this is limited to my account since this problem persists even when changing browser or device and also my friend’s account works just fine. What might be the problem?
A view like that is almost always caused by a very slow or poor internet connection.
If reloading the page does not resolve it, try reconecting to whatever source you are getting your internet connectivity from.
