Sometimes this happens if the user forgot the password to the old account, so they just make a new one, or if they logged in with Facebook and later created a separate account with a username/password.
Though sometimes it’s actually 2 different people, such as a student copying someone else’s observations in order to get class credit. Here’s what to do in that case: https://www.inaturalist.org/pages/fabricated_data
But, if it seems not to be of ill intent, I leave a comment or send the user a private message letting them know about the duplicate accounts/observations and how to delete them. If there are a lot of observations, without many duplicates, that need to be combined into a single account, I believe the staff can sometimes help out with that process, but that’d be assessed on a case by case basis.
You can also always directly email the staff at help@inaturalist.org to get support in questionable cases.

As far as I can tell, it’s not the same person, but perhaps family members on the same trip.
